What this website collects, why we collect it, and how to ask us to delete it. It covers activechiropractor.com only — the records the practice keeps about your care are handled separately.
Who we are

What we collect
Two things directly, plus whatever you choose to send through the forms described below.
- What you type into a form on this site — your name, and the email address or phone number you give us, along with any message and any preferred appointment time. The page you sent it from is recorded so we know what you were reading.
- Basic analytics about how the site is used — pages viewed, roughly where in the world the visit came from, the type of device and browser, and how you arrived. This is collected through Google Tag Manager and Google Analytics using cookies and similar technologies.
Why we collect it
Form details are used to answer you and to arrange an appointment. Nothing more. We do not sell them, rent them, or pass them to anyone for their own marketing.
Analytics are used to see which pages people actually read, so the site can be made more useful. They tell us about visits in aggregate, not about you personally.
Please do not use the appointment form or email to send us detailed health information — neither is a secure channel. Tell us that you would like an appointment and roughly what it is about, and we will take the rest at the practice or over the phone. The one exception is the new patient intake form on our online forms page, which is provided by an outside service specifically for collecting a health history.
Who else sees it
Enquiries submitted through this site are delivered into the system the practice uses to manage appointments and patient communication, which is operated by a third-party provider on our behalf. That provider may only process the information in order to provide that service to us.
Analytics data is processed by Google. Google’s own privacy policy governs what it does with it.
Beyond that, we disclose information only where the law requires it.
How long we keep it
Enquiries are kept for as long as we need them to answer you and to keep a record of our correspondence. If your enquiry becomes an appointment, the details you gave become part of your patient record and are held under the rules that govern those records rather than this policy.
Analytics data is retained according to the settings in our Google Analytics account.
Forms provided by other companies
Two of the forms on this site are operated by outside companies rather than by us, and information you type into them goes to that company as well as to the practice.
New patient intake form. The health history form on our online forms page is provided by Review Wave. What you enter there — including your health history and your signature — is submitted through their service.
Patient portal. The alternative intake link on the same page opens ChiroTouch, our practice management system.
Appointment requests. When you send an appointment request from this site, the name and contact details you give are stored in GoHighLevel, the system the practice uses to manage enquiries. If you tick the box to opt in to text messages, that choice is recorded with your enquiry; if you do not, it is recorded as a refusal and we will not text you.
Each of these companies handles the information under its own agreement with the practice. If you would rather not use them, call us on +1 (207) 571-8028 and we will take your details by phone or on paper when you arrive.
Cookies
This site sets cookies through Google Tag Manager and Google Analytics for the measurement described above. Every current browser lets you block or delete cookies in its settings, and most let you refuse them per site. Blocking them does not stop you using this website.
Asking us to delete your information
Write to us at info@activechiropractor.com, or call +1 (207) 571-8028, and ask us to delete the details you submitted through this website. Tell us roughly when you sent them so we can find them. We will confirm once it is done.
We may not be able to delete information that forms part of a patient health record, or that we are required by law to keep. If that applies, we will say so and explain why.
You can also ask us what we hold, ask us to correct it, or ask us to stop contacting you.
Children
This website is not directed at children and we do not knowingly collect information through it from anyone under 13. If you believe a child has sent us information through this site, contact us and we will delete it.
Other websites
This site links to websites run by other organisations. Once you follow one of those links, this policy no longer applies — the site you have arrived at has its own.
Changes to this policy
If this policy changes, the revised version will be published on this page. Please check it from time to time.
