Save time at the office by completing your paperwork securely online before your appointment.
New patient health history form
This tells us the history and current state of your health, and gives you somewhere to set out what you want to get out of care.
If the form will not load
The intake form is provided by an outside service, so occasionally it will not appear — usually because of a browser extension or a blocked script. You have two other options, and neither will delay your appointment.

Complete the same intake through our practice management system.

Your insurance card and photo ID, plus any recent imaging or reports relating to the problem. If you are not sure whether something is relevant, bring it.
