Fill in your new-patient paperwork at home rather than in the waiting room. It takes a few minutes and means Dr. Reiche has your history in front of him before you sit down.
New patient health history form
This tells us the history and current state of your health, and gives you somewhere to set out what you want to get out of care.
If the form will not load
The intake form is provided by an outside service, so occasionally it will not appear — usually because of a browser extension or a blocked script. You have two other options, and neither will delay your appointment.
